The South West London Health and Care Partnership is an organisation comprising the ones that deliver health and care in South West London's six Boroughs.
Through the NHS, local councils and the voluntary sector, this Partnership allows Kingston, Richmond and four more areas (Croydon, Sutton, Merton & Wandsworth) to work together and keep local people healthy.
In November 2016, the South West London Health and Care Partnership has created a South West London Sustainability and Transformation Plan (STP). In November 2017, they refreshed the strategy in order to develop Local Health and Care Plans. Each area faces different challenges in terms of health and social care, which these plans want to specifically address.
